Follow these simple steps to schedule and appointment.
Step One:
Ask the referral specialist at your Doctor's office to fax your referral to NPT. If they give you the (referral or prescription) call 904-277-4449 in Fernandina Beach or
904-879-1223 in Callahan.
Step Two: Have your insurance card available when you call us. NPT will gather the necessary information to call your insurance company to verify your physical therapy benefits. We will inform you of any deductible, co-insurance, co-pays, how many visits your insurance allows for therapy, if an authorization is required and any other pertinent insurance information.
Step Three:
Once your insurance has been verified we will schedule your initial evaluation. You will need to arrive at least 20 minutes early for paperwork, or you can print the paperwork from our website and bring it with you on your first visit. Please bring your insurance card, photo ID and prescription from your doctor on your first visit. Your first visit will last between 45 minutes to 1 hour. After your first visit follow up visits will be scheduled.
